About The Position
Amedisys Hospice, part of the Optum family of businesses, provides home health, hospice, and community-based services. The RN Case Manager coordinates compassionate, individualized hospice care by assessing patients, partnering with the interdisciplinary care team, and developing plans that support each patient’s needs. Patient coverage territories include Woburn, Waltham, and Watertown; orientation takes place at the care center in Woburn.
Requirements
- Current RN license specific to the state(s) assigned to work.
- Current CPR certification.
- At least 1 year of nursing experience.
- Valid driver's license, reliable transportation, and liability insurance.
Responsibilities
- Assess patients and collaborate with the care team to develop and implement a plan of care.
- Refer patients to other hospice care disciplines as needed, or document the rationale for not doing so.
- Develop, prepare, and maintain accurate, timely individualized patient care progress records according to care center policies.
- Record pain and symptom management changes and outcomes as appropriate.
- Teach patients, families, and caregivers appropriate patient care techniques.
- Regularly communicate with the clinical manager and care team.
- Perform on-call responsibilities and provide on-call services to patients and families as assigned.
- Perform other duties as assigned.
