Hospice RN Case Manager ($10k Sign-on Bonus)
About The Position
The Hospice RN Case Manager coordinates and provides high-quality hospice care to patients. The role includes developing individualized care plans, managing patient visits, collaborating with the interdisciplinary team, and ensuring compliance with hospice standards and regulations. The RN Case Manager is the primary point of contact for patients and families, providing compassionate support and clinical expertise throughout the hospice journey. The position works in patient homes and hospice facilities and requires local travel to patient locations. The role may involve varying home environments and weather conditions, prolonged periods of driving and patient care, and managing stressful situations with empathy and professionalism.
Requirements
- Graduate of an accredited school of nursing.
- Current RN license in the state of practice.
- Minimum of 2 years of nursing experience.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Proficiency with EMR systems and documentation.
- Ability to work independently and manage a caseload.
- Compassionate approach to patient and family care.
- Ability to manage stressful situations with empathy and professionalism.
Nice To Haves
- Hospice and Palliative Care certification.
- At least 1 year of experience in hospice or palliative care.
Responsibilities
- Conduct comprehensive assessments of hospice patients.
- Develop and implement individualized care plans.
- Coordinate care with physicians, social workers, chaplains, and other team members.
- Provide skilled nursing care and symptom management.
- Educate patients and families on disease progression and care expectations.
- Document all patient interactions and care provided in the EMR system.
- Participate in interdisciplinary team meetings and care planning.
- Ensure compliance with hospice policies and regulatory requirements.
- Respond to patient and family needs in a timely and compassionate manner.
