Requirements

  • Current and valid RN license in the state of CA.
  • Proficiency in conducting assessments, implementing care plans, and monitoring patient status.
  • Excellent interpersonal and communication skills to effectively interact with patients, families, and other healthcare professionals.
  • Ability to manage a caseload, coordinate care, and prioritize tasks.
  • Ability to analyze situations, identify problems, and make sound decisions in a timely manner.
  • Familiarity with relevant regulations and standards related to home health care.

Responsibilities

  • Conducts initial and ongoing assessments to determine the patient's needs and develop a personalized plan of care.
  • Collaborates with the patient, family, and other healthcare providers to establish goals and priorities.
  • Ensures the care plan aligns with the patient's preferences and addresses their physical, emotional, and social needs.
  • Coordinates the delivery of care, including delegating tasks to other healthcare professionals (e.g., physical therapists, home health aides).
  • Monitors the patient's progress and makes adjustments to the plan of care as needed.
  • Supervises and supports the home health team, ensuring quality and adherence to protocols.
  • Communicates regularly with patients, families, physicians, and other healthcare providers to facilitate care coordination.
  • Maintains accurate and complete documentation of patient assessments, care plans, and progress.
  • Participates in team meetings and case conferences to share information and collaborate on care.
  • Provides education to patients and families on their conditions, treatments, and home care procedures.
  • Addresses questions and concerns, offering support and guidance throughout the care process.
  • Serves as a patient advocate, ensuring their needs and preferences are considered.
  • Identifies and facilitates access to community resources and support services.
  • May be involved in discharge planning and transitioning patients back to their homes or other settings.
