The Nurse Case Manager (RNCM) creates, plans, and directs hospice care using the nursing process of assessment, intervention, implementation, and evaluation. The role collaborates closely with the Interdisciplinary Team and builds meaningful relationships with patients and their families. It requires a commitment to high standards of care and professional nursing clinical competency.

Requirements

• Current RN license in the State of Georgia.
• Utmost compassion and integrity.
• Valid driver’s license, insurance, and reliable vehicle.
• Excellent time management skills.

Nice To Haves

• 1–2 years of nursing experience preferred.
• 1–2 years of hospice experience preferred.
• Candidates with experience in skilled nursing, home health, acute care, med-surg, or ICU will be considered.

Responsibilities

• Create, plan, and direct hospice care using assessment, intervention, implementation, and evaluation.
• Collaborate closely with all members of the Interdisciplinary Team.
• Build meaningful relationships with patients and their families.
• Provide care consistent with high standards and professional nursing clinical competency.
