About The Position

Providence Care is hiring full-time Registered Nurses to provide case management services to patients enrolled in its hospice program. Patients receive care in their homes, assisted living settings, and long-term care facilities. The role focuses on end-of-life care, supporting patients and their families, and helping them create meaningful final memories.

Requirements

• Hold an unencumbered RN license in the state where applying.
• Have reliable transportation and a valid driver's license.
• Have a clean motor vehicle record (MVR).

• Experience in hospice or home health.

Responsibilities

• Provide case management to hospice patients and families, including symptom management, medication management, and collaboration with members of the interdisciplinary group (IDG).
• Oversee patient plans of care in collaboration with the physician.
• Conduct supervisory visits and provide oversight for LPNs and CNAs.
• Conduct start-of-care visits and collaborate with Care Navigators.
• Educate patients, families, and facility staff.
• Provide oversight to Home Health Aides and LPNs.
