About The Position

Moments Hospice is hiring a full-time, field-based RN Case Manager to provide compassionate hospice care. The RN Case Manager serves as the clinical lead for assigned patients and their families, managing symptoms, coordinating care, and providing support and guidance. The role offers a supportive setting, reasonable caseloads, direct access to leadership, and opportunities to shape a career. A company vehicle is available through the fleet car program for business and personal use, with fuel and insurance covered at minimal cost, based on eligibility.

Requirements

• Active Registered Nurse (RN) license in the state where the employee works.
• Proficiency with electronic medical records (EMR).
• Strong organizational, communication, and critical-thinking skills.
• Ability to work independently while maintaining high standards of care.
• Compassionate, patient- and family-centered approach.
• Valid driver's license and reliable vehicle with current insurance.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plans of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers about disease progression, symptom management, medications, and what to expect at end of life.
• Coordinate care across an assigned caseload and serve as the primary liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.
