RN Case Manager
About The Position
Moments Hospice is hiring a field-based RN Case Manager to provide compassionate, patient- and family-centered hospice care. The RN Case Manager serves as the clinical lead for patients and families, managing symptoms, coordinating care, and providing support and guidance during end-of-life transitions. The role includes reasonable caseloads, a supportive setting, access to leadership, and opportunities to shape a career. The schedule is Monday through Friday, 8:30 AM to 5:00 PM. The company offers a fleet car program with a company vehicle available for business and personal use, with fuel and insurance covered at minimal cost, subject to eligibility.
Requirements
- Active Registered Nurse (RN) license in the state where the employee works.
- Proficiency with electronic medical records (EMR).
- Strong organizational, communication, and critical-thinking skills.
- Ability to work independently while maintaining high standards of care.
- Compassionate, patient- and family-centered approach.
- Valid driver's license and reliable vehicle with current insurance.
Responsibilities
- Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
- Develop, implement, and update individualized plans of care in partnership with patients and families.
- Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
- Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life.
- Coordinate care across the caseload and serve as the primary liaison between patients, families, physicians, and the care team.
- Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.
