RN Case Manager
About The Position
Tennessee Quality Care Hospice is seeking an RN Case Manager for a field-based role serving Camden, Tennessee, and surrounding counties. The position manages a caseload of hospice patients, provides therapeutic nursing, coordinates care, and supports patients and their families in their homes. The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m., with no on-call duties. The RN Case Manager assumes primary responsibility for patients and families throughout the nursing process, including assessment, eligibility determination, admission processing, care planning, implementation, and evaluation. The role coordinates with physicians, the interdisciplinary team, and other agencies; reassesses patient needs; maintains records and care plans; and supervises Home Health Aide care as required.
Requirements
- Graduate from an accredited registered nursing program.
- Hold a registered nurse license in the state of practice.
- Possess current CPR and First Aid certifications.
- Be knowledgeable about quality assessment and assurance procedures.
- Have a valid driver's license and proof of insurance.
Responsibilities
- Manage a patient and family caseload, including assessment, eligibility determination, admission processing, planning, implementation, and evaluation of the nursing process.
- Collect and assess information about physical, psychological, social, and spiritual factors affecting patient and family needs, and initiate interventions by other interdisciplinary team members.
- Communicate with attending physicians, the interdisciplinary team, and other agencies to coordinate optimal care.
- Make regular visits to reassess patient needs and update the plan of care.
- Perform clinical pronouncement of death and support the family.
- Maintain accurate patient records, charts, progress notes, and reports, and prepare care plans in a timely manner.
- Supervise and document Home Health Aide care as required by Federal, State, and Company guidelines.
