About The Position

BrightStar Care is seeking a part-time RN Case Manager to join their growing team in the South Minneapolis Area. This role involves working 3 days per week, with a $3000 sign-on bonus. The RN Case Manager will be salaried and work collaboratively with a team of RNs and LPNs to complete assessments, re-assessments, and some skilled cares for home care private pay and veteran clients. As a non-Medicare agency, OASIS is not required. The team emphasizes a supportive, family-like environment focused on keeping clients safe, happy, and healthy in their homes. PTO accrues from day one, and paid mileage is provided for travel between clients and the office. The primary client base is in the Minneapolis Metro Area (Bloomington, Edina, Eden Prairie, etc.). Requirements

Active RN license in the state of Minnesota.
Previous experience in a home health or acute care setting is advantageous.

Responsibilities

Assess, plan, implement, and evaluate nursing care for patients in accordance with professional standards.
Collaborate with multidisciplinary team members to optimize patient care and outcomes.
Educate patients and their families on health maintenance and disease prevention.
Ensure accurate documentation and compliance with all regulatory guidelines.
