About The Position

Boost Home Healthcare is seeking an exceptional RN Case Manager to join their ground-floor team. This part-time role offers supplemental income with flexibility and direct access to leadership. The agency is Medicare-certified and ACHC-accredited, serving areas including Boston, Cambridge, Brookline, Newton, Needham, Wellesley, Quincy, and Milton. The position emphasizes true flexibility in choosing part-time hours, direct communication with the Director of Nursing, transparent pay with hourly rates and travel reimbursement, and a supportive clinical infrastructure with a small-team feel.

Requirements

RN (Registered Nurse) with active MA state license.
At least 1 year of experience in home health or case management preferred.
Expertise in administering IVs and medications.
Valid MA driver’s license and reliable transportation.
Able to commit 10 - 20 hours per week, flexible hours.
Familiarity with Medicare OASIS (training is available).
Strong critical thinking, communication, time management, and the ability to work independently.
Comfortable using EHR systems (experience with Kantime, Axxess, HCHB, or WellSky is a plus).
Valid driver’s license, auto insurance, and access to a dependable vehicle.

Responsibilities

Serve as the primary point of contact for a caseload of home health patients, ensuring seamless coordination of services.
Conduct initial evaluations and develop individualized plans of care.
Complete accurate OASIS assessments and timely documentation.
Coordinate care planning with physicians and the clinical team.
Provide education, resources, and emotional support to patients and their families to promote independence and wellness.
