About The Position

Boost Home Healthcare is seeking an exceptional RN Case Manager to join their ground-floor team. This part-time role offers supplemental income with flexibility and direct access to leadership. The agency is Medicare-certified and ACHC-accredited, serving areas including Boston, Cambridge, Brookline, Newton, Needham, Wellesley, Quincy, and Milton. The position emphasizes true flexibility in choosing part-time hours, direct communication with the Director of Nursing, transparent pay with hourly rates and travel reimbursement, and a supportive clinical infrastructure with a small-team feel.

Requirements

  • RN (Registered Nurse) with active MA state license.
  • At least 1 year of experience in home health or case management preferred.
  • Expertise in administering IVs and medications.
  • Valid MA driver’s license and reliable transportation.
  • Able to commit 10 - 20 hours per week, flexible hours.
  • Familiarity with Medicare OASIS (training is available).
  • Strong critical thinking, communication, time management, and the ability to work independently.
  • Comfortable using EHR systems (experience with Kantime, Axxess, HCHB, or WellSky is a plus).
  • Valid driver’s license, auto insurance, and access to a dependable vehicle.

Responsibilities

  • Serve as the primary point of contact for a caseload of home health patients, ensuring seamless coordination of services.
  • Conduct initial evaluations and develop individualized plans of care.
  • Complete accurate OASIS assessments and timely documentation.
  • Coordinate care planning with physicians and the clinical team.
  • Provide education, resources, and emotional support to patients and their families to promote independence and wellness.
