About The Position

Sanctuary Hospice is seeking a PRN Hospice RN Case Manager (RNCM) to join their dedicated nursing team providing exceptional end-of-life care throughout the community. In this rewarding role, you’ll deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. Requirements

  • Current unencumbered license as a Registered Nurse.
  • Minimum one (1) year experience as an RN in a medical surgical/acute care setting.
  • Must be computer proficient in typing and various programs, including background in EMR.
  • Possess and maintains current CPR certification if required by state.
  • Current driver's license, vehicle insurance, and access to dependable transportation.

Nice To Haves

  • Hospice experience is a plus.

Responsibilities

  • Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.
  • Partner closely with the Clinical Director to assess the eligibility and suitability of clients for specialized hospice services.
  • Deliver hands-on hospice nursing care, ensuring patient comfort and quality of life remain paramount.
  • Apply nursing skills by delivering top-notch care rooted in proven principles and techniques to ease pain and bring solace.
  • Empower patients and families through education, encouraging their active participation in creating personalized care plans that align with their goals.
  • Share responsibility for call duties.
