RN Case Manager
About The Position
Transitions At Home is a trusted provider of home health services, committed to delivering high-quality, compassionate care to patients in the comfort of their homes. We are seeking an experienced RN Case Manager to support our care team and play a vital role in coordinating and delivering patient-centered care. If you are a caring, compassionate, critical thinking RN who has experience in Home Health, ICU, ER and Skilled Nursing Homes who would appreciate and enjoy a flexible work schedule, independence, and appreciates a work family team culture, we would love to add you to our Transitions At Home Team. Transitions At Home Registered Nurse Case Managers provide skilled nursing care to include assessments, medications, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.
Requirements
- Active Wisconsin Registered Nurse License
- Two (2) years’ nursing experience
- CPR Certification
- A valid Wisconsin Driver's License and Car Insurance
Nice To Haves
- 1 year in Home Health (preferred)
Responsibilities
- Provide skilled nursing care to include assessments, medications, and treatments.
- Coordinate services.
- Counsel the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.
