About The Position

This role is for a Float RN Case Manager at Home Health Care, Inc., serving the Greater Twin Cities Metro area. Unlike traditional Case Managers who manage a single territory, the Float RN Case Manager will rotate through different parts of the service area based on patient census, staffing, and clinical needs. Assignments typically last approximately three months, allowing the nurse to learn the territory, manage patients, work with the local clinical team, and become a dependable resource before transitioning to a new area. The position requires a versatile nurse who can quickly adapt to new situations, learn quickly, communicate effectively, and take ownership of patient care. The Float RN Case Manager is expected to be a reliable resource that can be sent to any territory to support the team and ensure patients receive excellent care.

Requirements

  • Active Minnesota Registered Nurse (RN) license
  • Strong comprehensive assessment and clinical decision-making skills
  • Home health and OASIS experience strongly preferred
  • Confidence managing an independent patient caseload
  • Strong organization and time-management abilities
  • Excellent patient, caregiver, physician, and team communication
  • Flexibility to transition between territories as organizational needs change
  • Reliable transportation and willingness to travel throughout our service area

Responsibilities

  • Manage an assigned home health caseload within the current coverage territory.
  • Rotate throughout different portions of the service area based on clinical and staffing needs.
  • Complete OASIS Start of Care, Recertification, and Discharge assessments.
  • Develop and update individualized plans of care.
  • Perform comprehensive nursing assessments and skilled visits.
  • Provide wound care, medication management, patient education, and other skilled nursing services.
  • Monitor patient progress and recognize changes in condition.
  • Communicate with physicians regarding clinical concerns.
  • Coordinate with PT, OT, SLP, social work, and other interdisciplinary team members.
  • Help patients avoid unnecessary hospitalizations and remain safely at home.
  • Maintain timely and accurate clinical documentation in Homecare Homebase (HCHB).
