About The Position

The registered nurse/case manager plans, organizes, directs, and provides hospice care, drawing on nursing experience with an emphasis on community health education. The nurse uses community resources to plan and direct services that meet the needs of individuals and families in their homes and communities. The role includes on-call weekend responsibilities.

Requirements

• Graduate of an accredited diploma, associate, or baccalaureate school of nursing.
• Current state Registered Nurse license.
• Current CPR certification.
• Licensed driver with an automobile that is insured according to state and/or organization requirements and in good working order.
• Excellent observation, verbal and written communication, problem-solving, and basic math skills; nursing skills per competency checklist.
• Ability to lift, position, and/or transfer patients.
• Ability to lift supplies and equipment.

Nice To Haves

• At least two years of experience as a Registered Nurse in a clinical care, home health, or hospice setting is preferred.

Responsibilities

• Case manage and provide clinical direction to the interdisciplinary team, physician, and family.
• Implement, develop, and document the patient plan of care.
• Provide care using infection-control measures that protect staff and patients in accordance with OSHA.
• Ensure continuity of quality patient care with appropriate documentation.
• Monitor assigned cases for compliance with third-party payer requirements.
• Demonstrate commitment, professional growth, and competency.
• Promote agency philosophy and administrative policies.
• Perform assigned on-call responsibilities and provide on-call service to patients and families.
• Supervise hospice aides and licensed practical nurses.
• Perform other nursing duties assigned by management to support department goals and initiatives.
