The RN Case Manager provides case management for assigned patient populations, using clinical expertise, communication, and problem-solving skills to achieve optimal clinical and resource outcomes. The role supports cost-effective, coordinated care by reducing fragmentation and facilitating patient and family movement through the healthcare organization and across the continuum of care. The position assesses patient needs upon admission and at regular intervals to support care planning, discharge planning, and transitions. It coordinates safe transitions, including referrals for home health, hospice, durable medical equipment, medications, and other post-acute services. The RN Case Manager collaborates with physicians, nursing, social work, utilization review, and other care team members to align care and discharge plans, ensure appropriate resource use and length-of-stay management, and meet regulatory and payer requirements. The role also supports accurate patient-status and medical-necessity determinations, communicates discharge plans and financial implications to patients and families, identifies barriers to discharge, and helps prevent delays. The RN Case Manager maintains accurate documentation, participates in interdisciplinary meetings, coordinates patient transfers, and advocates for patients and families throughout care. The position also contributes to performance improvement, uses predictive analytics tools, and collects and analyzes care and utilization data to support quality, fiscal outcomes, and reduced readmissions.
