About The Position
Amedisys, part of the Optum family of businesses, provides home health, hospice, and community-based services. The RN Case Manager coordinates compassionate, individualized hospice care by assessing patients, partnering with the interdisciplinary care team, and developing plans that support each patient’s needs. The role includes on-call responsibilities and services to patients and families as assigned.
Requirements
- Current RN license specific to the state(s) assigned for work.
- At least 1 year of nursing experience.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
Responsibilities
- Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
- Makes referrals to other hospice care disciplines as needed or documents the rationale for not doing so.
- Develops, prepares, and maintains individualized patient care progress records accurately and promptly according to care center policies.
- Records changes and outcomes in pain and symptom management as appropriate.
- Teaches patients, families, and caregivers appropriate patient care techniques.
- Regularly communicates with the clinical manager and care team.
- Performs on-call responsibilities and provides on-call services to patients and families as assigned.
- Performs other duties as assigned.
