About The Position
Spectrum Healthcare Resources has a potential opportunity for an experienced Outpatient RN Case Manager to join the healthcare team at Joint Base Elmendorf-Richardson, Alaska. The role supports military patients as they navigate their healthcare needs through clinical care coordination. It is a full-time, Monday–Friday position, with hours from 7:30 AM to 4:30 PM.
Requirements
- U.S. citizenship.
- Associate Degree in Nursing or higher from an ACEN- or CCNE-accredited program.
- Current, unrestricted RN license from any U.S. jurisdiction.
- 3 years of RN Case Management experience within the last 4 years.
- Case Management certification (CCM, CDMS, CRRN).
- Current BLS certification.
Responsibilities
- Coordinate outpatient treatment plans and specialty referrals.
- Assess patient needs and develop individualized care plans.
- Collaborate with providers and community resources.
- Educate patients and coordinate ongoing care.
