RN Case Manager
About The Position
Moments Hospice is hiring an RN Case Manager to provide compassionate, patient- and family-centered hospice care. The role serves as the clinical lead for patients and their families, managing symptoms, coordinating care, and providing support and guidance during end-of-life transitions. The position is field-based and offers a supportive setting, reasonable caseloads, direct access to leadership, and opportunities to shape your career. The schedule is Monday through Friday, 8:30 AM to 5:00 PM. Compensation is $75,000–$90,000 per year. A fleet car program provides a company vehicle for business and personal use, with fuel and insurance covered, at minimal cost and based on eligibility.
Requirements
- Hold an active Registered Nurse (RN) license in the state where you work.
- Be proficient with electronic medical records (EMR).
- Have strong organizational, communication, and critical-thinking skills.
- Be able to work independently while maintaining high standards of care.
- Have a compassionate, patient- and family-centered approach.
- Have a valid driver’s license and a reliable vehicle with current insurance.
Responsibilities
- Complete initial and ongoing comprehensive assessments of patients’ physical, emotional, and psychosocial needs.
- Develop, implement, and update individualized plans of care in partnership with patients and families.
- Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
- Educate patients and caregivers about disease progression, symptom management, medications, and what to expect at end of life.
- Coordinate care across the caseload and serve as the primary liaison between patients, families, physicians, and the care team.
- Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.
