About The Position
Baywood Home Care is seeking a compassionate Registered Nurse (RN) Case Manager to coordinate care, support patients and families, and help individuals maintain their independence in the comfort of their own homes. Requirements
- Current Minnesota Registered Nurse (RN) license, required.
- Home health, case management, or transitional care experience, required.
- Strong assessment, communication, and care coordination skills.
- Must be 21 years of age or older.
Responsibilities
- Conduct patient assessments and develop individualized care plans.
- Coordinate services with physicians, therapists, caregivers, and community resources.
- Monitor patient progress and adjust care plans as needed.
- Educate patients and families on medications, treatment plans, and wellness strategies.
- Maintain accurate clinical documentation and regulatory compliance.
