About The Position

Moments Hospice is hiring an RN Case Manager to provide compassionate, patient- and family-centered hospice care. The RN Case Manager serves as the clinical lead for assigned patients, managing symptoms, coordinating care, and supporting patients and families through end-of-life care. The position is field-based and offers a supportive setting, reasonable caseloads, access to leadership, and opportunities to shape a career.

The schedule is Monday through Friday, 8:30 AM to 5:00 PM. The role includes conducting comprehensive assessments, developing individualized care plans, providing skilled nursing care, educating patients and caregivers, coordinating care, and participating in interdisciplinary group meetings.

Requirements

• Active Registered Nurse (RN) license in the state where the employee works.
• Proficiency with electronic medical records (EMR).
• Strong organizational, communication, and critical-thinking skills.
• Ability to work independently while maintaining high standards of care.
• A compassionate, patient- and family-centered approach.
• Valid driver's license and reliable vehicle with current insurance.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plans of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers about disease progression, symptom management, medications, and what to expect at end of life.
• Coordinate care across the caseload and serve as the primary liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.
