About The Position
Signature Healthcare at Home, part of The Pennant Group, is seeking a full-time, non-exempt Registered Nurse Case Manager to support our west-side Portland service area. This role is ideal for nurses who value autonomy, meaningful patient relationships, and the opportunity to deliver high-quality care directly in patients’ homes. As an RN Case Manager, you will manage a balanced caseload, provide skilled nursing interventions, coordinate interdisciplinary services, and guide patients and families through recovery, chronic-care management, and safe transitions. Requirements
- Current Oregon RN license (or ability to obtain).
- One year of nursing experience preferred; home-health experience a plus.
- Strong assessment, communication, and critical-thinking skills.
- Ability to work independently in a field-based role.
- Reliable transportation and valid driver’s license.

Responsibilities
- Complete comprehensive nursing assessments and develop individualized care plans.
- Provide skilled nursing interventions in patient homes, including wound care, medication management, chronic-condition support, and patient education.
- Manage a field-based caseload with strong organizational efficiency.
- Coordinate care with interdisciplinary team members to ensure seamless service delivery.
- Communicate effectively with physicians, caregivers, and internal team members.
- Maintain accurate, timely documentation in accordance with regulatory and organizational standards.
- Support safe transitions of care from hospitals, SNFs, and outpatient settings.
