About The Position

RN Case Manager Signature Healthcare at Home, part of The Pennant Group, is seeking a full‑time, non‑exempt Registered Nurse Case Manager to support our west‑side Portland service area. As an RN Case Manager, you will manage a balanced caseload, provide skilled nursing interventions, coordinate interdisciplinary services, and guide patients and families through recovery, chronic‑care management, and safe transitions. Requirements

  • Current Oregon RN license (or ability to obtain).
  • One year of nursing experience preferred; home‑health experience a plus.
  • Strong assessment, communication, and critical‑thinking skills.
  • Ability to work independently in a field‑based role.
  • Reliable transportation and valid driver’s license.

Responsibilities

  • Complete comprehensive nursing assessments and develop individualized care plans.
  • Provide skilled nursing interventions in patient homes, including wound care, medication management, chronic‑condition support, and patient education.
  • Manage a field‑based caseload with strong organizational efficiency.
  • Coordinate care with interdisciplinary team members to ensure seamless service delivery.
  • Communicate effectively with physicians, caregivers, and internal team members.
  • Maintain accurate, timely documentation in accordance with regulatory and organizational standards.
  • Support safe transitions of care from hospitals, SNFs, and outpatient settings.
