Requirements
- Licensed Physical Therapist Assistant (PTA) in Oregon
- Graduate of an accredited PTA program
- Current driver’s license and reliable transportation
- Ability to work independently in a field-based setting

- Home health experience
- Familiarity with OASIS and Medicare documentation
- Strong time management and clinical skills
Responsibilities
- Provide physical therapy treatments according to the plan of care established by the supervising PT
- Assist patients in improving mobility, strength, balance, and functional independence
- Observe and report patient responses to treatment
- Work closely with Physical Therapists, nurses, and the interdisciplinary care team
- Communicate patient progress and recommend adjustments to care plans
- Participate in care coordination and case discussions
- Instruct patients and families on home exercise programs, safety and fall prevention strategies, and proper use of assistive devices
- Complete accurate, timely clinical documentation
- Follow Medicare home health guidelines and Oregon state regulations
- Ensure compliance with company and regulatory standards
- Support patient-centered outcomes and satisfaction
- Help reduce hospital readmissions and promote independence
