About The Position

Registered Nurse (RN) Case Managers initiate and coordinate comprehensive nursing care for a diverse population, ranging from neonates to geriatric patients. As vital members of the multidisciplinary team, they work to improve patient outcomes and continuously elevate the quality, timeliness, and accessibility of care. The role manages acute and chronic care needs, coordinates care across hospital units and settings, and provides patient education and advocacy.

Requirements

• Active and unrestricted RN license; Alaska RN licensure must be obtained within one year of hire.
• Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), and Pediatric Advanced Life Support (PALS) certifications.
• Ability to work across multiple departments and provide care to diverse patient populations, including acute, chronic, neonatal, pediatric, and geriatric needs.
• Proficiency with electronic health records, with attention to accuracy and compliance.
• Ability to communicate effectively with patients, families, and interdisciplinary teams in a culturally sensitive and respectful manner.
• Familiarity with the challenges and resources of rural healthcare settings, including limited access to specialty services.

Nice To Haves

• Minimum of two years of clinical experience and exposure to multiple units preferred.

Responsibilities

• Demonstrate thorough knowledge of unit-specific protocols, workflows, referral, travel, and lodging processes, and patient care standards.
• Manage patients’ acute and chronic care needs while providing education and advocacy aligned with National Clinical Screening Guidelines.
• Deliver safe, efficient, evidence-based nursing care across hospital units and settings.
• Assess and prioritize patient needs and initiate care coordination.
• Collaborate with interdisciplinary teams to ensure coordinated, high-quality patient care.
• Maintain accurate and compliant electronic health records in accordance with documentation standards.
