DaVita Kidney Care•04651 - IKC Local East Texas-Louisiana-E TX, TX
•$37 - $49•Hybrid

About The Position

As a DaVita Integrated Kidney Care (IKC) Registered Nurse Case Manager, you will support complex patients with chronic kidney disease (CKD) and end-stage kidney disease (ESKD) as they navigate the healthcare system. Through medical record review and consultation, you will identify patients’ medical, social, emotional, educational, community, and financial needs and implement appropriate interventions. You will work autonomously and with the healthcare team to coordinate and facilitate quality, consistent, cost-effective care. The position is hybrid-remote, combining work-from-home telehealth with travel within an assigned geography for in-person visits at dialysis clinics and nephrology practices. The schedule is Monday through Friday, with flexibility to accommodate patient and nephrology partner availability. Responsibilities include building empathetic relationships with patients and families, coordinating care and transitions, managing complex or at-risk patients and ongoing needs, and establishing treatment plans with the care team. You will assess patients and identify unreported conditions or changes that could lead to adverse outcomes. DaVita IKC provides comprehensive care and complex chronic condition management to patients nationwide, including people in early stages of kidney disease, transitioning to dialysis, seeking a kidney transplant, or receiving dialysis. The organization describes its approach as value-based, person-centered, and comprehensive, with a mission to make integrated care standard for renal patients. The job posting identifies the role as part-time/per diem in its title, but also describes it as a full-time position in the position details.
