About The Position

DaVita Integrated Kidney Care (IKC) is seeking an experienced Registered Nurse Case Manager for a hybrid role supporting Morristown, Newton, and Succasunna, New Jersey. The position works autonomously and collaboratively with the healthcare team to coordinate and facilitate quality, cost-effective care for patients with end-stage renal disease (ESRD) and chronic kidney disease (CKD), while minimizing fragmentation in the healthcare delivery system. The nurse coordinates care among patients, families, physicians, providers, care teams, the community, and the Regional Operations Manager.

DaVita IKC is DaVita Inc.’s integrated care division. Its teams of nurses, care coordinators, nurse practitioners, and business professionals integrate and manage care for patients with late-stage CKD and ESRD. Requirements

• Associate degree in nursing (ADN) from an accredited school of nursing; a three-year diploma from an accredited diploma program may be substituted for a nursing degree.
• Current Registered Nurse (RN) license in the state of practice.
• Continuing education credits maintained as required by the state of practice.
• At least five years of clinical nursing experience.
• Demonstrated knowledge and understanding of data and of managing clinical, financial, and patient satisfaction outcomes.
• Demonstrated experience and effectiveness in a change-agent role.
• Demonstrated knowledge and understanding of continuous quality improvement (CQI) techniques.
• Previous experience in healthcare performance coaching.
• Current CPR certification.
• Ability to adapt personal practice patterns to new or electronic processes and increased productivity expectations related to Capella implementation.
• Current driver’s license in the state of employment, a positive driving record, and ability to meet insurance coverage requirements.
• Basic computer skills and proficiency in Microsoft Word and Outlook.
• Functional proficiency with DaVita-specific clinical software programs, including Capella, required within 90 days of employment.
• Home office with internet connectivity of at least 1 Mbps upload and 1 Mbps download.

Nice To Haves

• Bachelor of Science in Nursing (BSN).
• At least three years of renal nursing experience.
• Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) certification.
