About The Position

Moments Hospice is hiring an RN Case Manager to provide compassionate hospice care and serve as the clinical lead for patients and their families. The role involves managing symptoms, coordinating care, and supporting families through end-of-life transitions. The position is field-based and offers a supportive setting, reasonable caseloads, direct access to leadership, and opportunities to shape a career.

The schedule is Monday through Friday, 8:30 AM to 5:00 PM. The compensation range is $75,000 to $90,000 per year. The company describes its fleet car program as providing a company vehicle for business and personal use, with fuel and insurance covered, at minimal cost to the employee, subject to eligibility.

Requirements

• Active Registered Nurse (RN) license in the state where the employee works.
• Proficiency with electronic medical records (EMR).
• Strong organizational, communication, and critical-thinking skills.
• Ability to work independently while maintaining high standards of care.
• A compassionate, patient- and family-centered approach.
• Valid driver's license and reliable vehicle with current insurance.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plans of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers about disease progression, symptom management, medications, and what to expect at end of life.
• Coordinate care across the caseload and serve as the primary liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.
