About The Position
The RN Case Manager plays a critical role in supporting patients and their families throughout the continuum of care. This position focuses on care coordination, discharge planning, and ensuring patients receive appropriate resources and services tailored to their individual needs. The RN Case Manager collaborates closely with physicians, nurses, and interdisciplinary teams to promote safe transitions and high-quality patient outcomes.
Requirements
- Active Registered Nurse (RN) license
- Associate Degree or Nursing Diploma (BSN preferred)
- Minimum of 2 years of recent (within the last 3 years) acute care experience
Nice To Haves
- Experience in ICU, CCU, Med-Surg, LTAC, or Case Management preferred
Responsibilities
- Guide patients and families through care programs, including orientation, treatment plans, insurance benefits, and discharge criteria
- Coordinate patient, family, and caregiver education to encourage active participation in care planning
- Develop and implement individualized care and discharge plans based on patient needs and goals
- Perform psychosocial assessments and document findings accurately
- Collaborate with interdisciplinary teams to ensure seamless, patient-centered care
- Communicate effectively with physicians and healthcare team members regarding patient progress and discharge readiness
- Advocate for patient safety and appropriate discharge planning
- Maintain clear, concise, and timely documentation
