About The Position

We are seeking an experienced Registered Nurse (RN) – Case Manager for a 13-week assignment at a leading acute care hospital in Mission Viejo, CA. This role is ideal for RNs with a strong background in acute care case management and care coordination.

Requirements

• Active California RN License
• Minimum 2 years of recent acute care Case Management/Care Coordination experience.
• Previous travel nursing experience is required.
• Strong communication, organizational, and care coordination skills.
• Willingness to float to the other facility as needed.
• Availability for weekend rotation.

Responsibilities

• Coordinate patient care and discharge planning.
• Collaborate with physicians and the interdisciplinary healthcare team.
• Assess patient needs and arrange appropriate post-acute services.
• Ensure continuity of care and timely discharge planning.
• Maintain accurate documentation and case management records.
• Advocate for quality, patient-centered care.
