RN Case Manager
About The Position
Hearth Hospice is seeking a full-time Hospice Registered Nurse Case Manager to provide end-of-life nursing care throughout the community, covering Rhea and Bledsoe Counties. The role involves delivering hands-on hospice care, coordinating services, and guiding patients and families through the hospice journey as part of an interdisciplinary care team. The position is scheduled Monday through Friday, 8:00 a.m. to 4:30 p.m., and includes shared call duties. Requirements
- Current, unencumbered Registered Nurse license.
- At least one year of RN experience in a medical-surgical or acute-care setting.
- Computer proficiency, including typing and use of various programs, with experience using electronic medical records.
- Current CPR certification if required by the state.
- Current driver’s license, vehicle insurance, and access to dependable transportation.

Responsibilities
- Respond to clinical referral information and address inquiries and requests.
- Work with the Clinical Director to assess clients’ eligibility and suitability for hospice services.
- Provide direct nursing care and support to patients, prioritizing comfort and quality of life.
- Deliver nursing care using established principles and techniques, including care to ease pain.
- Educate and advocate for patients and families, supporting their participation in personalized care plans.
- Share responsibility for call duties.
