About The Position

Lighthouse Hospice is seeking a full-time Hospice Registered Nurse Case Manager (RNCM) to provide end-of-life care throughout the community. The role delivers hands-on hospice nursing care, coordinates services, and guides patients and families throughout their hospice journey as part of an interdisciplinary care team. The territory includes Mercer, Burlington, Camden, Gloucester, and Salem County. The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m., with shared call duties.

Requirements

• Hold a current, unencumbered Registered Nurse license.
• Have at least one year of RN experience in a medical-surgical or acute care setting.
• Be proficient in typing and computer programs, including electronic medical records (EMR).
• Maintain current CPR certification if required by the state.
• Have a current driver’s license and vehicle insurance, and access to dependable transportation.

Responsibilities

• Respond to clinical referral information and address inquiries and requests.
• Partner with the Clinical Director to assess client eligibility and suitability for hospice services.
• Provide direct nursing care and support patients’ comfort and quality of life.
• Deliver nursing care using proven principles and techniques to help ease pain and provide comfort.
• Educate and advocate for patients and families, encouraging their participation in personalized care plans.
• Share responsibility for call duties.
