About The Position

This role is for an RN Case Manager in home health, focusing on patient care within the community. Unlike traditional bedside nursing, this position offers more ownership and continuity of care, allowing the nurse to follow a patient's progress from admission through discharge. The case manager will get to know their patients, coordinate with physicians and therapists, educate families, and identify changes in condition to intervene earlier and prevent hospitalizations. The territory covers Southwest Minneapolis and the Inner Metro area, including Edina, St. Louis Park, Richfield, and Hopkins, with visits generally concentrated within a 35-mile radius.

Requirements

  • An active Minnesota Registered Nurse (RN) license
  • Three or more years of RN experience
  • Reliable transportation for visits throughout your assigned territory
  • Experienced nurses from hospital, acute care, rehabilitation, skilled nursing, and other clinical environments are encouraged to connect with us.

Responsibilities

  • Manage an assigned home health patient caseload
  • Complete OASIS Start of Care, Recertification, and Discharge assessments
  • Perform comprehensive nursing assessments
  • Develop and update individualized plans of care
  • Provide skilled nursing visits
  • Monitor patient progress throughout the episode of care
  • Perform wound care, medication management, and other skilled nursing services
  • Educate patients and caregivers on diagnoses, medications, treatments, and recovery
  • Identify and communicate changes in patient condition
  • Coordinate directly with physicians and healthcare providers
  • Collaborate with PT, OT, SLP, social work, and interdisciplinary team members
  • Help patients safely transition home following hospitalization or rehabilitation
  • Work proactively to prevent avoidable emergency department visits and rehospitalizations
  • Complete timely documentation using Homecare Homebase (HCHB)
