About The Position

Amedisys is hiring a full-time, day-shift Registered Nurse Case Manager for home health. The role supports patients healing at home by assessing their needs, coordinating with the care team, and developing and implementing plans of care. The case manager promotes patient health and independence through education and appropriate rehabilitative measures, supervises LPNs and home health aides, and keeps patient documentation and progress updates timely and accurate. The position also includes staff education, on-call responsibilities as assigned, and participation in clinical development and continuing education. Pay is listed at $75,000–$85,800 and may vary by location, job-related knowledge, skills, and experience.

Requirements

• At least one year of clinical experience as a Registered Nurse; candidates with less than one year require approval by the VP of Clinical.
• Current RN license specific to the state or states assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• Candidates with less than six months of clinical RN experience must participate in the RN Intern program.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other disciplines based on identified patient needs, or document the rationale for not making referrals.
• Promote patient health and independence through teaching and appropriate rehabilitative measures, including instruction in self-care techniques.
• Supervise LPNs and home health aides.
• Complete documentation accurately and on time.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform on-call responsibilities and provide on-call services to patients and families as assigned.
• Participate in clinical development and continuing education programs.
• Perform other duties as assigned.
