RN Case Manager
About The Position
Community LIFE is seeking a Registered Nurse Case Manager (RNCM) to provide holistic care to a frail elderly population in patients’ homes and the day center medical clinic. The role is part of a compassionate, interdisciplinary team supporting older adults to live independently with dignity and purpose. The schedule is Monday through Friday, 7:30 AM to 4:00 PM, plus an on-call rotation, and the position is based in Rostraver, Pennsylvania, with local travel to patients’ homes. Community LIFE is a Program of All-Inclusive Care for the Elderly (PACE), helping seniors remain in their homes while receiving comprehensive medical and social support. Its team-based approach aims to provide each participant with personalized, high-quality care in a welcoming environment.
Requirements
- Graduate of an accredited nursing program.
- At least one year of experience with frail elderly populations.
- Active Pennsylvania RN license.
- Valid driver’s license.
- Strong clinical knowledge and critical-thinking skills.
- Understanding of geriatric care needs.
- Excellent communication and organizational skills.
- Ability to work independently and as part of a team.
- Familiarity with managed care, quality assurance, and utilization review.
- Basic computer proficiency, including Windows, email, and word processing.
Nice To Haves
- Experience in home care, hospice, long-term care, or community health is preferred.
- Experience in patient education and interdisciplinary collaboration is a plus.
Responsibilities
- Conduct in-home assessments and develop individualized care plans.
- Coordinate nursing and home care services.
- Collaborate with a multidisciplinary team in the day center clinic.
- Participate in care planning meetings and maintain thorough documentation.
- Travel locally to visit patients in their homes.
