About The Position

Serenity Hospice is seeking a dedicated and compassionate RN Case Manager to provide care, comfort, and support to hospice patients and their families. The role develops individualized care plans, conducts regular assessments, and communicates with family members and the healthcare team as needed. The position oversees hospitals throughout Morris County, New Jersey.

Requirements

• Current New Jersey RN license in good standing.
• One year of hospice experience required.
• One year of hospital-setting experience required.
• Willingness to travel to assigned areas.
• Reliable transportation with liability insurance.
• Strong communication and interpersonal skills.

Responsibilities

• Complete an initial assessment of the patient and family to determine hospice care needs.
• Regularly reevaluate patients' nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the healthcare team, patient, and family to direct continuous, coordinated care.
• Counsel patients and families in meeting end-of-life needs.
• Initiate appropriate preventative nursing procedures.
• Communicate with physicians regarding patients' needs and report changes in patient condition.
• Supervise Hospice Aides and LPNs.
