About The Position

Serenity Hospice is seeking a dedicated and compassionate RN Case Manager to provide exceptional care, comfort, and support to hospice patients. The role develops plans based on individual needs, conducts regular check-ups, and communicates with family members as needed. The RN Case Manager oversees the Atlantic County areas.

Requirements

• Current New Jersey RN license in good standing.
• Willingness to travel to assigned areas.
• Reliable transportation with liability insurance.
• Strong communication and interpersonal skills.

Nice To Haves

• Hospice experience preferred.

Responsibilities

• Complete an initial assessment of the patient and family to determine hospice care needs.
• Regularly reevaluate patients’ nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the healthcare team, patient, and family to direct coordinated, continuous care.
• Counsel patients and families in meeting end-of-life needs.
• Initiate appropriate preventative nursing procedures.
• Communicate with the physician regarding patient needs and report changes in the patient’s condition.
• Supervise Hospice Aides and LPNs.
