About The Position

Hospice of North Alabama is seeking a dedicated Registered Nurse Case Manager to provide expert, patient-centered hospice care to patients and families in the East Morgan County coverage area. The role is based at the Cullman, Alabama office and serves patients in the Decatur, Alabama area. The schedule is Monday through Friday, 8 a.m. to 5 p.m., with participation in an on-call rotation as required by the local branch.

The RN Case Manager assesses patients’ physical, emotional, and psychological needs; develops care plans that align with patients’ wishes and goals; directs nursing care; manages pain and symptoms; and educates and supports families and caregivers. The nurse collaborates with an interdisciplinary team and maintains accurate, timely documentation. Hospice of North Alabama emphasizes individualized care in the home and offers independent work with team support.

Requirements

• Graduate of an accredited nursing school.
• Current licensure to practice in the state of operation.
• At least one year of nursing experience.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Licensed driver with an automobile insured according to state or organization requirements and in good working order.
• Current CPR certification in states where required.

Nice To Haves

• Three or more years of nursing experience preferred.
• Experience in hospice or a similar setting preferred.

Responsibilities

• Assess and monitor patients’ physical, emotional, and psychological needs.
• Create hospice care plans aligned with patients’ wishes and goals.
• Direct nursing care, including administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patients’ families and caregivers.
• Collaborate with the interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in the local branch’s on-call rotation as required.
