RN Case Manager | Hybrid 50% WFH | Gainesville Area
About The Position
The RN Case Manager plans, manages, and coordinates care for an assigned caseload of patients receiving supportive care services. This primarily telephonic role provides comprehensive, compassionate care in collaboration with an interdisciplinary team. The position is hybrid, with 50% of the work involving home care around the greater Gainesville area and 50% involving work from home and phone calls with patients and families. The schedule is Monday through Friday, 8 a.m. to 5 p.m., with no weekends. Compensation is a base salary of $68,000–$72,000, plus paid mileage for driving. The position includes a full benefits package and 23 days of annual PTO.
Requirements
- At least three years of experience as a Registered Nurse.
- Registered Nurse license in the state of Florida, up to date.
- Current CPR certification.
- Associate Degree or Bachelor's Degree in Nursing from an accredited nursing program.
Nice To Haves
- One year of hospice or palliative care experience preferred.
Responsibilities
- Conduct initial and ongoing assessments of patients’ physical, functional, psychosocial, and environmental needs.
- Coordinate and implement individualized plans of care aligned with patient and caregiver goals.
- Facilitate goals-of-care discussions, including advance care planning.
- Provide telephonic support, guidance, and education to patients and caregivers.
- Collaborate with interdisciplinary team members, including medical providers and social workers.
- Monitor patient conditions and communicate symptom management needs.
- Ensure timely and accurate documentation in accordance with organizational standards.
- Participate in interdisciplinary group (IDG) meetings and care planning reviews.
- Comply with applicable local, state, and federal regulations.
- Maintain professional relationships with facilities and community partners.
- Participate in professional development and mentor new staff as needed.
- Perform other related duties as assigned.
