About The Position

Serenity Hospice is seeking a dedicated and compassionate RN Case Manager to provide care, comfort, and support to hospice patients and their families in the Lakewood, New Jersey area. The RN Case Manager develops plans based on individual needs, conducts regular check-ups, and communicates with family members as needed. The role includes assessing and reassessing nursing needs, coordinating care with the healthcare team, counseling patients and families about end-of-life needs, and supervising Hospice Aides and LPNs.

Requirements

• Current New Jersey RN license in good standing.
• Willingness to travel to assigned areas.
• Reliable transportation with liability insurance.
• Strong communication and interpersonal skills.

Nice To Haves

• Hospice experience preferred.

Responsibilities

• Complete an initial assessment of the patient and family to determine hospice care needs.
• Regularly reevaluate patients’ nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the healthcare team, patient, and family to direct coordinated and continuous care.
• Counsel patients and families in meeting end-of-life needs.
• Initiate appropriate preventative nursing procedures.
• Communicate with the physician about patient needs and report changes in the patient’s condition.
• Supervise Hospice Aides and LPNs.
