Requirements
- Current and active Registered Nurse (RN) license in Maine
- Minimum of 2 years of nursing experience
- Experience with electronic medical records (EMR) systems
- Reliable transportation and ability to travel throughout the Brunswick service area

- Home health, care management, or case management experience preferred
Responsibilities
- Complete comprehensive patient assessments and ongoing evaluations
- Develop, implement, and manage individualized care plans
- Coordinate home health services and community resources
- Monitor patient progress and adjust care plans as needed
- Collaborate with physicians, therapists, caregivers, and family members
- Educate patients and families on health management and wellness strategies
- Maintain accurate and timely clinical documentation
- Ensure compliance with all regulatory and agency standards
