About The Position

Amedisys is seeking a registered nurse case manager to provide home health care. The role involves assessing patients, working with the care team to create and implement plans of care, promoting patient health and independence, and supervising LPNs and HHAs. The position is full-time, day shift, and includes assigned on-call responsibilities. Compensation is $74,802.00–$93,506.40 annually; pay may vary by location, job-related knowledge, skills, and experience.

Requirements

• At least one year of clinical experience as a Registered Nurse (RN); candidates with less than one year require approval by the VP Clinical.
• Current RN license specific to the state(s) assigned.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• Candidates with less than six months of clinical RN experience must participate in the RN Intern program.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement plans of care.
• Refer patients to other disciplines based on identified needs, or document the rationale for not making a referral.
• Promote patient health and independence through teaching and appropriate rehabilitative measures, including instruction in self-care techniques.
• Supervise LPNs and HHAs.
• Complete documentation accurately and on time.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform assigned on-call responsibilities and provide on-call services to patients and families.
• Participate in clinical development and continuing education programs.
• Perform other duties as assigned.
