About The Position

Amedisys is hiring a registered nurse case manager to provide home health care and help patients heal at home. The role involves assessing patients, collaborating with the care team on plans of care, promoting patient health and independence, and supervising LPNs and home health aides. The listed pay is $47.95–$59.94 base rate/pay per visit after orientation. Compensation may vary by location, job-related knowledge, skills, and experience.

Requirements

• At least one year of clinical experience as a registered nurse; candidates with less than one year require approval by the VP Clinical.
• Current RN license specific to the state(s) where assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• Candidates with less than six months of RN clinical experience must participate in the RN Intern program.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement plans of care.
• Make referrals to other disciplines based on identified patient needs, or document the rationale for not making a referral.
• Promote patient health and independence through teaching and appropriate rehabilitative measures, including helping patients learn self-care techniques.
• Supervise LPNs and HHAs.
• Complete documentation accurately and on time.
• Regularly communicate patient progress to the clinical manager and care team.
• Plan and provide staff education.
• Perform assigned on-call responsibilities and provide on-call services to patients and families.
• Participate in clinical development and continuing education programs.
• Perform other duties as assigned.
