About The Position

Our client, a healthcare company, is seeking an RN Case Manager for its Vienna, West Virginia location. The role manages high-risk members through comprehensive assessment, individualized care planning, care coordination, and ongoing evaluation of clinical, functional, and fiscal outcomes. The Case Manager works with providers and other care professionals, arranges appropriate levels of care and non-medical support, promotes health and illness prevention, and advocates for patient privacy and confidentiality.

Requirements

WV or compact Registered Nurse license.
Home health experience.
OASIS experience.
HCHB experience.
Associate or bachelor's degree in nursing or a related field.
Current, unrestricted state Registered Nurse license.
Ability to analyze complex medical information and make rational decisions.
Case Management certification.

Responsibilities

Perform comprehensive assessments of high-risk members.
Collaborate with primary care providers to implement individualized, comprehensive care plans with specific interventions designed to engage members.
Identify and continuously reassess appropriate, cost-efficient levels of care.
Arrange non-medical support, such as housing and transportation, to help ensure compliance with treatment plans.
Coordinate care transitions as needed.
Coordinate with physicians, social workers, discharge planners, and claims professionals to transition patients to appropriate levels of care.
Engage specialty resources as needed to achieve optimal resolution.
Maintain detailed records of clinical, functional, and fiscal outcomes during the management process.
Identify opportunities for health promotion and illness prevention.
Prevent adverse patient occurrences when possible and intervene quickly when prevention is not possible to minimize poor outcomes.
Act as a patient advocate by protecting privacy and confidentiality.
