About The Position

Heart to Heart Hospice provides compassionate care to patients with life-limiting illnesses and their loved ones. The RN Case Manager assesses patients’ needs, implements and updates individualized plans of care, and coordinates care with patients, caregivers, physicians, and other healthcare professionals. The role also includes on-call responsibilities, timely documentation, and compliance with applicable laws, regulations, and agency policies.

Requirements

Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Current State Registered Nurse License.
Current driver’s license, reliable transportation, and ongoing valid and current auto liability insurance.

Nice To Haves

Two years of experience as an RN, preferably in a healthcare or clinical setting.

Responsibilities

Conduct initial and ongoing comprehensive assessments of how terminal disease affects patients’ physical, psychosocial, environmental, and activities-of-daily-living needs, including grief risk and cultural, spiritual, verbal, and non-verbal considerations.
Implement individualized plans of care and recommend revisions as needed.
Coordinate care among patients, caregivers, physicians, and healthcare professionals.
Attend Inter-Disciplinary Group meetings and coordinate and participate in patient care planning, prioritization of needs, and care coordination with the group.
Perform assigned on-call responsibilities and provide on-call service to patients and families.
Submit all documentation in a timely manner.
Perform other duties as required.
Ensure overall compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
