About The Position
Holisticare Hospice is seeking a full-time Hospice Registered Nurse Case Manager to provide end-of-life care to patients and families throughout Philadelphia County and Bucks County. The role delivers hands-on hospice nursing care, coordinates services, and guides patients and families throughout their hospice journey as part of an interdisciplinary care team. Care is grounded in compassion, clinical excellence, comfort, dignity, and connection.
Requirements
- Current, unencumbered Registered Nurse license.
- At least one year of RN experience in a medical-surgical or acute-care setting.
- Computer proficiency, including typing and use of various programs; background with electronic medical records (EMR).
- Current CPR certification if required by the state.
- Current driver's license, vehicle insurance, and access to dependable transportation.

Responsibilities
- Respond to clinical referral information and address inquiries and requests with care and expertise.
- Partner with the Clinical Director to assess client eligibility and suitability for hospice services.
- Provide direct nursing care and support to patients, prioritizing comfort and quality of life.
- Deliver nursing care using established principles and techniques to help ease pain and provide comfort.
- Educate and advocate for patients and families, encouraging participation in personalized care plans aligned with their goals.
- Share responsibility for call duties.
