About The Position
We are looking for a full-time, civilian Registered Nurse for the Case Manager position in support of the Traumatic Brain Injury Department. The qualified Registered Nurse will have a minimum of 1 year of direct Case Management experience with individuals and groups with complex, catastrophic, chronic or life changing injuries or TBI.
Requirements
- Minimum of 1 year of direct Case Management experience with individuals and groups with complex, catastrophic, chronic or life changing injuries or TBI.
- BLS certification
- ACLS certification
- PALS certification
Responsibilities
- Function as a member of an interdisciplinary team to meet individual and family comprehensive health needs.
- Duties involve planning, coordinating, facilitating, and evaluating health care, serving as an advocate for patient options and services.
- Participate in all phases of the Case Management Program (CMP) and ensure that the CMP meets established case management (CM) standards of care.
- Develop treatment plans including preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions to ensure continuity of care toward the goal of optimal wellness.
- Provide appropriate health care instruction to patient and/or caregivers based on identified learning needs.
