About The Position

Aegis Healthcare is expanding its Home Health and Hospice Nursing Teams! This is a full-time position, Monday-Friday, 8 am-5 pm, with one 24-hour on-call shift per month. The compensation is $40-$45 per hour, and the location is East Valley, AZ (Mesa / Apache Junction / Gilbert / Chandler). As a Home Health & Hospice Nurse Case Manager, you will provide exceptional care to patients in their homes, conduct comprehensive assessments, collaborate on care plans, manage care delivery, maintain records, and act as a resource for families.

Requirements

Active Registered Nurse (RN) license in Arizona (or state of practice).
Effective verbal, written, and time management skills.
Valid driver's license, reliable transportation, and proof of current auto insurance.
Ability to perform patient transfers with a full range of movement.
Current CPR certification and negative TB test (we can assist).
Valid AZ Fingerprint Clearance Card (we can help obtain it).
All Aegis Healthcare employees must be able to pass a criminal background check.

Responsibilities

Conducting comprehensive physical, social, and psychological assessments.
Collaborating with patients, families, and the Interdisciplinary Group to create individualized care plans.
Managing care delivery and maintaining meticulous patient records.
Acting as a resource for families, making proactive referrals, and fostering a holistic care experience.
