About The Position

The RN Case Manager assesses the impact of terminal illness on patients’ physical, psychosocial, and environmental needs and activities of daily living. The role implements and recommends revisions to individualized plans of care, coordinates care with patients, caregivers, physicians, healthcare professionals, and the Inter-Disciplinary Group (IDG), and completes documentation and on-call duties as assigned. The position also ensures compliance with applicable laws, Medicare regulations, and personnel policies and procedures.

Requirements

Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Current State Registered Nurse license.
Current driver’s license, reliable transportation, and ongoing valid and current auto liability insurance.

Nice To Haves

Two years of experience as an RN in a healthcare or clinical setting is preferred.

Responsibilities

Conduct initial and ongoing comprehensive assessments of how terminal disease affects patients’ physical, psychosocial, and environmental needs and activities of daily living, including grief risk and cultural, spiritual, verbal, and non-verbal needs.
Implement the individualized plan of care and recommend revisions as necessary.
Collaborate on care with patients, patient caregivers, physicians, and healthcare professionals.
Attend Inter-Disciplinary Group (IDG) meetings; coordinate and participate in patient care planning and prioritization of needs; and collaborate with the IDG to promote care coordination.
Perform assigned on-call responsibilities and provide on-call service to patients and families.
Submit all documentation in a timely manner.
Perform other duties as required.
Ensure overall compliance with local, state, and federal laws, Medicare regulations, and established personnel policies and procedures.
