About The Position
Seva Home Health is a locally owned and growing company seeking an RN Case Manager to cover Tulare County and surrounding communities. Our goal is to continually serve our community by providing high-quality, compassionate care in the intimate setting of our patients' homes. This is a per diem position.
Requirements
- Minimum of 1 year of experience as a Registered Nurse with a valid license required
- Current CPR/BLS Certification
- Reliable transportation required
Nice To Haves
- Experience with Kinnser strongly preferred
- Comfortable driving to patient homes for patient visits
- Prior experience with Palliative Care preferred
- Prior experience in Home Health, Hospice, Infusion, or Home Care preferred
Responsibilities
- Initiate, develop, and implement the patient Plan of Care (POC). Revise as needed in consultation with the patient, physician, and other team members when appropriate.
- Establish patient-specific goals and incorporate therapeutic, preventative and rehabilitative nursing services.
- Responsible for the oversight and delivery of quality, patient-centered care rendered in the patient’s home or living facility.
- Communicate necessary revisions to patient status and/or needs with the entire care team
- Regularly consult, advise and reevaluate patient status with patient, family, patient representative and other team members as appropriate regarding safety, plan and outcome of care and discharge planning.
- Provide complete, accurate and timely documentation consistent with the plan of care
