About The Position
Community Care, Inc. is seeking a compassionate Registered Nurse to provide home health care to clients in the Westmoreland and Fayette County area. The role involves delivering personalized care in patients’ homes and building meaningful connections while working in a supportive home setting. The schedule is Saturday and Sunday, with hours varying, on a day shift. The role is based in New Stanton, Pennsylvania.
Requirements
- Current Registered Nurse (RN) license in Pennsylvania.
- Graduation from an accredited nursing program.
- At least 1 year of clinical nursing experience.
- Current CPR certification.
- Reliable transportation and a valid driver’s license.
- Caring, independent approach and a passion for pediatric care.
Nice To Haves
- Home health experience preferred.
Responsibilities
- Conduct skilled nursing visits in patients’ homes, including assessments, reassessments, medication administration, IV therapy, and treatments.
- Develop and implement individualized care plans.
- Educate patients and caregivers on managing conditions and medications.
- Collaborate with a multidisciplinary team to ensure continuity of care.
- Document visits and patient progress accurately and promptly.
